Fear World is the fourth Elseworld to appear in parts around Metropolis. It is an Earth where Batman failed to stop the City of Fear incident and Scarecrow succeeded in saturating the world in his Fear Toxin.

Before Suicide Squad: Kill the Justice League Incident[]
Much like on Earth-1, Scarecrow would cause the City of Fear incident by coming up with a plan to fully expose all of Gotham to his improved Fear Toxin. However, unlike his Earth-1 counterpart, this world's Batman failed to stop Scarecrow, causing his plan to succeed in full, and not only expose Gotham but ultimately the entire world to the Fear Toxin.
Unlike most of the other Elseworlds, Brainiac chose to invade this world's Gotham as opposed to Metropolis, and didn't really hold any interest in this world once he did invade. After he wiped out all the inhabitants, he left the world to fall apart on its own. Resulting in broken, floating land with lava pouring out of the chunks.
Suicide Squad: Kill the Justice League Incident[]
Lawless ended up in this world, infiltrating the Skull Ship in order to free Green Lantern. She left various messages around the broken landscape for the Suicide Squad in case she died on mission. Learning of Green Lantern being alive again, Waller sent the Squad to the Fear World in order to aid Lawless in her plan to rescue Green Lantern.

- The layout of Gotham City is notably similar to Knight, as the Bleake Island GCPD is next to Arkham Asylum.
- Fear World is the first of the Elseworlds introduced in the "Season of" updates for Suicide Squad: Kill the Justice League to not be an alternate version of Metropolis, instead being an alternate Gotham City, with the primary locations shown in this world being the remaining parts of the city itself along with what little is left of Arkham Asylum.
- Zorro is replaced by Hego as the movie hero playing at the theater.
- Lois Lane noted that the pieces of this world look "nightmarish" and give off residual Fear Toxin. This implies the architecture was rusted and twisted by the Toxin, warping it.
- She also mentions everyone feels the absence of Batman more because of it. A subtle tribute to Kevin Conroy.
- Jack Ryder is advertised heavily on this world, suggest that his and Vicki Vale's popularity and methodology are swapped.
- Arkham appears to be more central to Gotham in this world that the others, seeing as parts of the island fragments advertise Arkham products.
- Plus GCPD is closer to the Asylum and there is a lack of walls on the main Arkham island. Meaning that villains had a harder time breaking out and Gotham was safer.
